A useful birth plan for an Australian hospital is a clear one-page set of preferences, not a script that labour has to follow. It should help your midwife, doctor, support person, and hospital team understand what matters to you while leaving room for your health, your baby's health, and your own choices to change on the day.

Start the plan after you know the basics of your care setting. Public hospitals, private hospitals, birth centres, shared care, planned inductions, and booked caesareans can all have different routines, visitor rules, equipment, and newborn-care policies. Ask your midwife or doctor which options are realistic at your hospital before you treat any template as final.
The next section is communication. Write how you prefer information to be shared, whether you want time to ask questions before non-urgent procedures, whether a support person should help you remember details, and any words, triggers, trauma history, cultural practices, religious needs, disability access, or privacy requests that would help you feel safer.
Pain relief preferences belong in order, not as a yes-or-no list. You can write that you would like to try breathing, movement, massage, water, heat, TENS, gas, morphine or pethidine where offered, or an epidural, then add what you would prefer if the first plan is not enough. It is normal to change your mind in labour.
Birth positions can be a short list of options you want to try. Standing, kneeling, side-lying, squatting, all-fours, using the bed upright, or using a stool or mat may or may not fit your labour, pain relief, monitoring, and hospital setup. Write what usually helps your body feel calm, then let the team guide safety if circumstances change.
Include a caesarean or assisted-birth backup section even if you hope not to need it. Note whether you want your support person present if possible, whether you want explanations during the procedure, whether you would like photos if allowed, who announces the baby, and whether skin-to-skin or early contact should happen as soon as it is safe.
The newborn-care section should cover the first hour and the first day. Write your preferences for skin-to-skin, delayed cord clamping if suitable, who cuts the cord, feeding support, vitamin K, hepatitis B immunisation, newborn checks, weighing, bathing, and whether baby should stay with you whenever possible. Ask your hospital how these routines work locally.
Feeding plans should be specific and kind to your future self. You can say whether you hope to breastfeed, express colostrum, formula feed, combination feed, avoid bottles unless medically needed, or get lactation help early. The goal is to help staff support your plan, not to make feeding feel like a test you pass or fail.
